Hours and Location:  The School Based Health Center located at Houghton Lake High School is NOW open every day.  Monday, Wednesday and Friday from 9 am-3 pm and Tuesday and Thursday from 10 am -4 pm.  Walk in appointments are accepted from 11 am - 1 pm daily but appointments are encouraged for routine care.  FREE Sports Physicals are available by appointment only.

Target Population:  The SBHC now has a pediatric focus with a target population of children and youth ages 10-21.  Dependent children of this population are also eligible for services.  Adults are no longer eligible for services through the School Based Health Center.   

Medicaid Services: The SBHC can assists families with Medicaid Enrollment. 

ALL STUDENTS MUST HAVE A CONSENT FORMS ON FILE TO BE SEEN IN THE CENTER. 

 

Parental Consent is required for all patients under 18. Students from Collins Elementary and the middle school can be seen at the clinic if they are escorted by a parent. Visitors to the clinic from other buildings must sign in at the main office.

Services Provided:  Although crisis intervention and emergency care do not require parental consent, regular medical services require a signed consent from a parent or guardian before services are provided. The following services are available from the Health Center:

·        Physical exams/sports physicals/well child checks

·        First Aid

·        Diagnosis and management of chronic illnesses/diseases

·        Sick care/minor illness (ex. sore throats or earaches)

·        Basic lab tests including urinalysis, glucose, rapid Strep test

·        Dispensing of over the counter medications (ex. Tylenol, Ibuprofen, Tums, etc.)

·        Dispensing of prescription medications with physician consent

·        Student, family and community health education

·        Crisis intervention

·        Individual and Group counseling

·        Needs Assessments

·        Referral for specialty care such as vision, hearing, dental, orthopedics, etc.

 

Current Michigan Law allows for confidential (no parental consent) services to minors in these areas:

·        Counseling and referrals for mental health, physical/sexual abuse, neglect, substance abuse

·        Referral for prenatal care

·        Diagnosis and treatment of sexually transmitted infections

·        Gynecological services

 

 

Services that will NOT be provided at the Health Center include:

·        Birth control pills or devices cannot be dispensed or prescribed

·        Abortion counseling or referral

·        Prenatal care

 The School Based Health Center is a community service provided by MidMichigan Health Services with funding from the Michigan Department of Education and the Michigan Department of Community Health.

  • Don't share drinks, food, or anything that goes in or on the mouth
